Pathways was developed using LifeShare’s two decades of experience. A collaborative model created to impact culture, capacity, and choice, Pathways supports people with developmental disabilities on their journey towards self-actualization and independence.
Realizing the importance of giving individuals, their families and guardians a voice and choice in services and support, LifeShare pioneered Pathways to promote a more self-directed model of care.
- Pathways complements health plan integrated care management models by facilitating collaboration among stakeholders, including state agencies, managed care organizations, providers, facilities, individuals, families, and guardians.
- Pathways is designed to eliminate the silos that exist within the service delivery system through bridging network providers, defining quality outcome measures, and maximizing technology to improve communication, reduce costs, and enable increased quality of life.
- The five Pathways to independence include physical health; emotional and behavioral health; supporting families and self-direction; community living; and employment and day support services.
- LifeShare has successfully worked with hundreds of individuals with developmental disabilities using Pathways methods. Pathways is also applicable to various long-term services such as Child Welfare and Elder Care.
LifeShare helped develop the nation's first fully-integrated Medicaid Managed Care program for people with developmental disabilities in the state of Kansas and is working on the development of similar programs in other states.
